Wellies on the Ground is a free mental health outreach and support service provided by The Cresco Foundation. 

Please complete this form with the individual's knowledge and permission. The information provided will be reviewed by a member of the Cresco Mental Health Team to determine whether Wellies on the Ground is an appropriate service.

Referrals should normally only be made with the individual's knowledge and permission. Where information must be shared without consent because of safeguarding, serious risk or another lawful basis, this should be clearly documented.

What happens next?

Once your referral has been received, it will be reviewed by a member of the Cresco Mental Health Team.

If the individual meets the geographical and service criteria, a member of the team will contact them to introduce Wellies on the Ground, discuss their circumstances and establish how the service may be able to help.

Where appropriate, an initial outreach session will then be arranged.

Wellies on the Ground is completely free to access.
